Primary Health Care Program

The Primary Health Care (PHC) program works with clinic sites across the state to provide comprehensive primary and preventive health care to eligible men, women and children.

These services include:

  • Diagnosis and treatment of common health problems
  • Emergency services
  • Family planning services
  • Preventive health services, including immunizations
  • Health education and counseling
  • Diagnostic testing, such as X-rays and lab services

Who Is Eligible for Services?

The program is available to anyone who:

  • Is a Texas resident;
  • has an income level at or below 200% of the federal poverty level; and
  • is not eligible for other programs or benefits that provide the same services.

How Do I Apply for Services?

Search for Primary Health Care providers online at 211Texas.org or call 211 and ask for the nearest provider.

Applications and eligibility forms are available at clinic sites, or speak with staff by phone or in person to determine if you qualify for services. In certain cases, services are available the same day you apply.
