  • Dialysis treatments
  • Access surgery
  • Drugs
  • Travel to transplant or dialysis services (mileage reimbursement)
  • Medicare premiums

Effective Jan. 1, 2023, the Comprehensive Immunosuppressive Drug Coverage for Kidney Transplant Patients Act will provide lifetime Medicare coverage of immunosuppressant drugs (ISDs) for kidney transplant recipients who are eligible for Medicare. How to Become a Provider

Providers can enroll with KHC through the Provider Enrollment and Management System (PEMS). Visit the Texas Medicaid & Healthcare Partnership (TMHP) Provider Enrollment website for more information on the enrollment process. To fully enroll and start receiving payments, submit the Texas Identification Number (TIN) Application - Form 4109 (PDF) as an attachment in PEMS. You can receive payments electronically by completing and emailing the TIN Application and Direct Deposit Authorization – Form 74-176 (PDF) to SHProviderRelations@hhs.texas.gov.
