Contractor Enrollment Process and Requirements

There is one standard contract and enrollment application for all Community Services providers. The application packet is comprised of a number of different forms and required documents.


Home and Community-based Waiver Instructions, Technical Guide and Review Criteria

The Centers for Medicare and Medicaid Services (CMS) has developed a technical guide for 1915(c) waiver applications (PDF). The technical guide provides states with information to assist in developing a viable 1915(c) waiver. Providers may review the guide to assist them in providing comments to HHS on 1915(c) waivers. Licensing Renewals and Contacts

Please do not use this site to renew a current license. Notification of renewals will be received through the Texas Unified Licensure Information Portal (TULIP). As of January 1, 2019, all renewals must be completed through TULIP. Completed applications for license renewal must be submitted 45 days before the current license expires.

An applicant who submits an application for renewal later than the 45th day before the expiration date of the license must pay a late fee of an amount equal to one-half the basic renewal fee.
