The Texas Health and Human Services Commission has released the Texas electronic Induced Termination of Pregnancy Reporting system. You can access the ITOP Reporting system on the HHS site. All abortion procedures performed, and complications diagnosed or treated on or after June 1, 2019 in Texas, must be reported through the electronic ITOP Reporting system. Paper forms will not be accepted.
Senate Bill 8 (85th Legislature, Regular Session) and House Bill 13 (85th Legislature, 1st Called Session) require this system. It replaces the following paper forms previously used to report abortion procedures and complications:
- Induced Abortion Report Form
- Abortion Complications Reporting
- Medical Emergency Abortion Incident Report Form
- Third Trimester Induced Abortion Certification Form
Older versions of Internet Explorer, including versions 8, 9 or 10, do not support the ITOP Reporting system but it can be accessed using the following browsers:
- Microsoft Edge (Internet Explorer 11)
- Google Chrome
- Apple Safari
- Mozilla Firefox
